Please describe the purpose of your organization, its history and mission and current programs, the population your organization serves, the budgetary needs your organization has, whether your organization is a non-profit organization or a charitable organization under Section 501(c)(3) of the Internal Revenue Code, and other relevant matters. Please explain why your organization is requesting a grant, the amount of the grant you are seeking, what the grant intends to accomplish, how your goals and use of the grant support the purposes of the Potomac River Boathouse Foundation, Inc., a timeline and budget for the project for which you are requesting the grant, your proposed staffing of the project, the names of individuals who will direct or oversee the project and report to the Potomac River Boathouse Foundation, Inc. regarding your use of the funds, and how you will measure the results of the project. I acknowledge and certify to the best of my knowledge that, if a grant is awarded pursuant to this proposal, (1) grant funds will not be used for any purposes other than those for which the grant was made, (2) my organization will submit periodic written reports to the Potomac River Boathouse Foundation, Inc. regarding use of the grant funds, (3) my organization will provide a final written report and accounting of how grant funds were used, (4) grant funds will not be used for any unlawful purposes, and (5) the Potomac River Boathouse Foundation, Inc. has the authority to withhold or recover grant funds if such funds are misused.
